Lodge 88 Lionfish

By registering my child(ren) with the Lodge 88 Lionfish Swim Team , I agree to participate (or allow my child(ren) and family members to participate) in the Lodge 88 Lionfish Swim Team , and hereby release Lodge 88 Lionfish Swim Team , its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the H Lodge 88 Lionfish Swim Team , including travel to and from training sessions, swim meets or other scheduled team activities.

I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participating in the Lodge 88 Lionfish Swim Team program and other classes.

LIABILITY RELEASE AND INDEMNIFICATION FORM

I, the undersigned participant and parent, request voluntary participation for minor to participate in Lodge 88 Lionfish Swim Team Activities. sponsored by Lodge 88 Lionfish Swim Team which begins August 1st and ends December 31st are hereinafter referred to as the “activity”.

I consent to my/minor’s participation in the activity and acknowledge that the minor and I fully understand my/minor’s participation may involve risk of serious injury or death, including losses which may result not only from my/minor’s own actions, inactions or negligence, but also from the actions, inactions, or negligence of others, the condition of the facilities, equipment,or are as where the event or activity is being conducted,and/or the rules of play of this type of event or activity. I understand that if I have any risk concerns, I should discuss the risks associated with my participation with the activity coordinators and event staff, before I sign this document and before the activity begins.

Release – Minor’s Rights:

In consideration of allowing Minor Participant to participate in this Lodge 88 Summer Swim team event, I hereby release and hold harmless Lodge 88, HTX Swims and any members of its board of directors, and its officers, employees, members, volunteers, other participants, and agents (collectively, the “Released Parties”), of and from, and do discharge and waive, any and all claims, demands, losses, damages, and liabilities that Minor Participant may have or sustain with respect to any and all damage and/or injury, of any type, arising out of his or her participating in this Lodge 88 event. I also agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

Release – Parents’/Guardians’ Rights:

In consideration of allowing Minor Participant to participate in this USA Swimming event, I hereby release and hold harmless the Released Parties, of and from, and do discharge and waive, any and all claims, demands, losses, damages, and liabilities that I may have or sustain with respect to any and all damage and/or injury, of any type, arising from Minor Participant’s participation in this USA Swimming event. I also agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

I certify that my/minor is in good health and have no physical condition that would prevent participation in this activity. Furthermore, I agree to use my/minor’s personal medical insurance as a primary medical coverage payment if accident or injury occurs. I consent to emergency medical treatment in the event such care is required.

Indemnification by Parent/Guardian:

The undersigned parent/guardian further agrees to indemnify, save and hold harmless the Released Parties from any and all claims, demands, losses, damages and liabilities for indemnities, contribution or otherwise with respect to any damage and/or injury, of any type, arising from Minor Participant’s participation in this Lodge 88 Program.

The Lodge 88 Lionfish Swim team may take photographs (individual and in groups) of swimmers under the age of 18 that may include your child during their membership in the team. As the parent/ guardian, I allow photos and videos to be taken for training and coaching purposes or for the club’s social media (Instagram and website) or team banquet/party purposes. This includes any of the staff members or volunteers designated at events, practice or swim meets.

It takes Hundreds of volunteer hours to have a successful Summer League season. In line with the mission of Lodge 88 we require our swim team members to volunteer throughout the summer season. Volunteer hours can be achieved through practice, swim meet, and administrative means. 

VOLUNTEER POLICY

• For each Lionfish family an adult member is required to fulfill the volunteer hour requirement.
• Refusal to fulfill the volunteer jobs can result in the swimmer being removed from the meet.
• If you are unable to fulfill the job you have volunteered for, you must make arrangements with the team's volunteer coordinator ( )
• The Board reserves the right to change your volunteer responsibility at the time of the meet based on the need to fill all positions.
• By not fulfilling volunteer hours your swimmers registration for the 2025 season may be denied. 

I/We hereby give approval for the participation of my child (children) in any and all swim team activities on the Swim Team, and I/we assume all risk and hazards incident to such participation including transportation to and from said activities, and waive, release, absolve, indemnify and agree to hold harmless the Memorial-Spring Branch Swimming Conference Inc., its Members, all participating teams, officers, organizers, coaches, supervisors, directors, volunteers, participants, and persons or parents involved in any way, including transporting participants to and from such activities, from claims arising out of injury to my/our child, and/or damages to or loss of property. 

I/We acknowledge that I/We understand that there is currently no way to guarantee the prevention of the spread of COVID-19. I/We are voluntarily choosing to participate in the Memorial Spring Branch Swimming Conference, Inc. and understand the risks associated with the usage of public facilities. I/We agree to abide by governmental guidelines regarding personal health monitoring, social distancing, and group size as well as directives of lifeguards and pool management. I/We hereby release and hold harmless MSBSC from any and all liability arising from COVID-19 illness or injury arising from swimming activities. 

I/We also certify that my/our child has not violated Memorial-Spring Branch Swimming Conference Inc. eligibility rules by training or competing with a USA swim team or organization or YMCA swim team or organization after March 3rd of the current swim season. 

I/We understand that any violation of this eligibility rule will result in my/our child being ineligible for this league for one year and the forfeiting of each meet in which he/she competed for the aforementioned swim club.

I/We confirm that I/we have read the Memorial-Spring Branch Swimming Conference, Inc. Sportsmanship Policy and agree to follow the policy by promoting good sportsmanship.
